Media & Photo Release

By submitting this form or participating in any program, volunteer activity, event, or initiative hosted by The DS&P Foundation, Inc., you acknowledge and agree to the following:

1. Permission to Use Image & Likeness

You grant The DS&P Foundation, Inc. permission to:

• Photograph, film, record, and/or otherwise capture your image, voice, name, likeness, statements, participation, and related media content

• Use, publish, display, reproduce, distribute, and share such content for purposes related to the Foundation’s charitable mission, including but not limited to:

• Foundation website content
• Social media posts
• Newsletters and email communications
• Printed materials
• Event promotions
• Educational resources
• Donor updates
• Grant reports
• Community impact materials
• Marketing and public relations materials

2. Rights and Ownership

• All photographs, videos, recordings, and related media content captured by or on behalf of The DS&P Foundation, Inc. may become the property of The DS&P Foundation, Inc.

• You waive the right to inspect or approve the final materials, edits, captions, designs, or uses of the content before publication or distribution.

3. No Compensation

You understand and agree that:

• No payment, royalty, or other compensation will be provided for the use of your image, voice, likeness, or participation.

• The use of such content supports the charitable, educational, outreach, and community impact mission of The DS&P Foundation, Inc.

4. Withdrawal of Consent

• You may revoke this permission by contacting The DS&P Foundation, Inc. in writing.

• Revocation applies only to future use of content where reasonably possible and does not apply to materials already created, published, printed, distributed, or used before receipt and processing of the written revocation request.

5. Minors

• If the participant is under the age of 18, a parent or legal guardian must provide consent on behalf of the minor participant.
